Nova Scotia Prenatal Record

The Reproductive Care Program of Nova Scotia (RCP) has been producing and distributing a standardized prenatal record to prenatal care providers in Nova Scotia free of charge, for over twenty-five years. Recently, we have revised the prenatal record based on valuable feedback we received from prenatal care providers around the province, changes to clinical practice guidelines, the latest evidence and expert consensus. In addition, information was gathered through piloting of a draft at three obstetrical clinics in the province.

The Nova Scotia Prenatal Record was launched through a mass mail-out to all prenatal care providers the week of August 6th, 2007. The packages included copies of the prenatal record together with associated materials. The Nova Scotia Prenatal Record 2007: Three pages for each record. The prenatal record pages are in duplicate form and can be ordered free of charge from RCP. Outdated Prenatal Records

All out-dated, unused prenatal records prior to July 2007 should be destroyed. The Nova Scotia Prenatal Record dated July 2007 should be used for all new prenatal patients.

There was a package of information from the Prenatal Screening program at the IWK Health Centre mailed out to all prenatal care providers. This package included:

  • A Prenatal Screening information brochure
  • Referral form for Advanced Maternal Age
  • First Trimester Maternal Serum Testing requisition
  • Second Trimester Maternal Serum Testing requisition
  • Family History Questionnaire
